Family Health Centers of San Diego (FHCSD) is looking for an CPT/Health Navigator. CPT duties will be: Using proper sterilization procedures, perform venipunctures, finger sticks, and specimen collections for use in the treatment and diagnosis of disease. May serve as a mentor in providing guidance to staff. Health Navigator duties will be: supporting the planning, implementation, and delivery of health education topics for our patients. You provide individual and group education, and participate in community events and outreach activities. You support the mission of FHCSD by creating culturally and linguistically appropriate curriculums, planning programs, delivering education, and evaluating effectiveness. You have a passion for community health promotion, for serving the medically underserved, and you understand how to communicate health topics in meaningful, culturally sensitive, and relevant ways.

Responsibilities: CPT:

  • Follow all established guidelines for venipuncture, finger stick, heel stick, and collection of body fluid specimens.
  • Complete all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards.
  • Identify problems that may adversely affect test performance or reporting of test results.
  • Any identified problems require immediate notification to technical CLS supervisor.
  • Workstation and laboratory areas are kept clean in accordance with departmental and regulatory standards.
  • Maintain lab supplies and materials at appropriate stock levels.
  • Monitor and adhere to laboratory safety per the FHCSD chemical hygiene plan and safety processes.
  • Perform specimen handling, storage of specimens and processing according to Laboratory Procedure Manual.
  • Follow the Laboratory's procedures for specimen handling and processing, reporting, and maintaining records of patient test results.
  • Input order entry, pending log and cancellation of specimens into the computer tracking system.
  • Perform clerical duties related to reporting of results and distribution in a timely manner.
  • Perform Point-of-Care Waived Test according to Laboratory Procedure Manual.
  • Perform other duties as assigned.

Responsibilities: Health Navigator:

  • Assists in project data collection, evaluation activities, and required reporting. Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards; in addition, documentation complies with program/grant requirements.
  • *Individually and/or with a team, plans, creates, and develops health education topic outlines and curriculums, incorporating various learning techniques and formats (i.e., group discussion, lectures, role-play, etc.). Reviews and assists in determining the linguistic and cultural appropriateness of materials, teaching aids, activities, methods, strategies, programs, etc. Review the efficacy of materials and programs and provides positive, action-oriented feedback. Assists with the translation and/or revisions of translated materials as appropriate.
  • *Performs group/individual assessments and counseling and provides patients with current, accurate health information. Ensures maximum utilization of schedules by proactively managing visits, including scheduling follow-up appointments, calling no-shows, working with recall technicians, PSRs, and Case managers.
  • *Performs other duties as assigned.
  • *Provides group classes and staffs community events for health education topics, ensuring correct material is delivered in culturally appropriate and relevant ways.
  • Perform other duties as assigned.

Requirements: CPT:

  • High School Diploma, GED Equivalent or Certified Phlebotomy Technician Certificate from an approved DHHS training facility Required.
  • Current California license or certificate as a Phlebotomy Technician.
  • Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advanced Resuscitation Training.
  • 0 - 4 years of experience performing venipuncture, finger stick, heel stick, and collection of body fluid specimens.

Requirements: Health Navigator:

  • 2 years of experience delivering health education, preferably in a community health setting.
  • Bachelor's degree in Health Education, Health Science, Nutritional Science, Public Health, Communications, or closely related field required. Master's degree preferred.
  • Specific Certifications may be required or preferred depending on assignment (i.e., CA HIV Test Counselor, limited CPT certification for waived testing, lactation certification, etc.)
  • Bilingual in Spanish/English or other language may be required depending on assignment.

In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits or potential bonuses.

$21.00 - $26.80

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
