Family Health Centers of San Diego (FHCSD) is looking for a Women's Case Management Coordinator. Working as a member of a dynamic care team the Women's Case Management Coordinator is responsible for assisting with the daily operations for Women's Case Management work. Duties include training staff, identifying areas for improvement, collecting departmental data, and creating reports. In addition, the Coordinator serves as a subject matter expert for referrals, assisting in resolving difficult referrals and working closely with health plans to obtain updates and settle authorization discrepancies and inconsistencies.

Responsibilities:

*Collects and disseminates performance and status data; serves as an information resource for Women's Case Managers; provides guidance and resolution options to staff regarding complex and difficult cases.

*Follow-up on 100% of all positive results for cancer within 72 hours. 100% of positive DETEC cases must be followed up within two months.

*Meets minimum productivity standard daily and reports data to supervisor daily adhering to processes and guidelines.

*Performs other duties as assigned.

*Provides direction to Case Managers to best manage patient cases, ensures updated referral information, and assists with internal audits.

*Provides supportive services such as eligibility determination, enrollment assistance, and patient education and support, including event support.

*Routinely tracks and manages patients to ensure timely care needs are completed and follow-up care is received as indicated and documented.

*Trains and directs staff on work processes, maintains updated training materials, and assists with audit preparation.

*Works on special projects, assists with reporting needs and supervises as needed.

Requirements:

*At least 3 years of experience in healthcare, with 1 year of experience in women's health.

*Bachelor's degree in public health, healthcare administration, business administration, or closely related field preferred.

*Ability to multi-task, be flexible, ensure accuracy, and meet changing priorities in a fast-paced, high-workload environment.

*Basic analytical skills and ability to create useful, actionable reports from data.

*Basic knowledge of medical terminology.

*Bilingual in English/Spanish required.

*Intermediate computer literacy (i.e., use system to manage and schedule appointments, and access electronic medical record information).

*Intermediate interpersonal and customer service skills.

*Intermediate knowledge of medical referral processes and insurance/payor requirements.

*Intermediate organizational skills, attention to detail, time-management skills, and strong motivation to meet deadlines and achieve goals.

*Intermediate written and verbal communication skills.

*Strong analytical and problem-solving skills.

In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits or potential bonuses.

$23.00 - $32.10

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
