Job Roles

  • Develop an assessment of the patient, in collaboration with the interdisciplinary treatment team, the patient, family members and significant others, whenever possible. The goal of the assessment is to highlight the patient's strengths, limitations, and internal/external supports and service needs to optimize the patient's functional status that has been affected by the COVID-19 Pandemic.
  • Develop treatment plans and set achievable treatment goals with the patient/family in collaboration with the internal interdisciplinary team members. This will include psychosocial problems in the interdisciplinary treatment plan. Perform ongoing assessments and updated treatment plans are done when necessary or in response to the changing needs as health status changes. Address the unique needs of clients with respect to disease status and psycho-social support impacted by the COVID-19 Pandemic.
  • Collaborate with other service providers in reassessing patient needs for services/programs and entitlements. Educate the patient and/or families of the available organizational and community services and assisting them in establishing the appropriate referrals/services based on their unique needs.
  • Develop and contribute to a resource file of FHCSD and community social service and assistance programs and will refer the patient to needed services.
  • Advocate for the patient both internally as well as to external health care providers and community service groups when it is in the best interest of the patient. When appropriate, the Social Worker will encourage the patient to advocate for themselves.
  • Make rapid assessments and develop crisis management plans to maintain patient safety and determine the need for admission to acute, short term and/or long-term placements. Utilize advanced practice skills, professional judgement and expertise regarding the patient's self-care capability, family support system, and the individual health care needs and possible consequences.
  • Provide education related to FHCSD and community resources, entitlements, Advance Directives/Living Will and will refer patients/families to the appropriate area for any further identified health education needs beyond the scope of practice.
  • Grant funded program. This position is funded through 03/31/2023; continuation of employment is dependent upon availability of and/or renewed funding.
  • Perform other duties as needed.

Education/Certifications/Licenses/Registrations

  • Bachelor's degree in Social Work, Psychology, Public Health or equivalent field required.
  • Master's degree in Social Work, Psychology, Child Development, Public Health preferred.
  • Or equivalent combination of education and experience that provides the skills, knowledge, and ability to perform the essential job duties, and which meets any required state or federal certification requirements.
  • Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 3 years; No more than 2 violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving with 7 years.

Experience/Specialized skills (including Language)

  • 3 years' experience in health care.
  • Bilingual in English/Spanish preferred.
  • Ability to maintain good interpersonal interactions with clients and co-workers as a member of a multi-disciplinary community health team with a diverse multi-cultural population.
  • Ability to multi-task, be flexible, ensure accuracy, and meet changing priorities in a fast-paced, high-workload environment.
  • Ability to provide support, structure, empathy, sound judgment, insight into human behavior and family relations.
  • Basic computer literacy ability to comply with department needs and expectations (i.e., electronic medical record documentation, obtaining background information and reports on patients, following up on appointments, etc.).
  • Knowledge of behavior modification theories.
  • Knowledge of current psychosocial assessment and counseling theories and best practices, and ability to translate best practices into meaningful, culturally appropriate resources.
  • Knowledge of the community resources, health and social service systems in San Diego County and skill in establishing working relationships with community partners.

In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits or potential bonuses.

$22.00 - $27.47

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
